AN APPOINTMENT FOR ALL TYPES OF TATTOO SERVICES.

1. You should be aware that this procedure is a permanent change in your skin and your body;

    

2. You understand that Hayley Inks reserves the right to refuse your tattoo project for any reason it deems valid; reserves See also reserve

    

3. You authorize Hayley Inks to photograph your tattoo for her portfolio, which she can share publicly;

    

4. You understand that no refunds are possible for deposits or tattoos;

    

5. You must notify Hayley Inks at least 72 hours before an appointment if you have to move it for any reason, otherwise you will lose your deposit;

    

6. You understand that the studio has no means to identify if you are allergic to the elements or ingredients that will be used for your tattoo;

    

7. You agree to take care of the tattoo by following the instructions given to you by Hayley Inks;

    

8. You understand that you will increase your risk of getting an infection if you do not follow the instructions given to you regarding the maintenance of your tattoo;

    

9. You release Hayley Inks from all claims, expenses, damages and responsibilities;

    

10. You confirm that the information you provided when scheduling an appointment is accurate and truthful;

     

11. You confirm that an appointment request can be canceled at any time if the proposed project or the information provided in the appointment form does not reflect the values or criteria of Hayley Inks.
